The Flourishing Roadmap
One-off professional planning package
Who it's for:
Families who want a concrete, usable plan to guide supports and progress goals.
What's included:
- Pre-meeting intake (30–45 minutes) to gather goals, routines and supports
- One focused planning session (1–2 hours) with family and key workers (remote or limited in-person)
- A written Roadmap (3–6 pages) with clear actions, measurable short-term milestones, suggested NDIS wording and simple progress measures
- One follow-up review (30 minutes) within 6–8 weeks
Timeline: Typically 2–3 weeks from intake to delivery (faster for urgent cases)
Pricing: From $800 (private pay / self-managed)
Family Navigation Coaching
Ongoing support
Who it's for:
Families who want regular help to use plan funds, report progress, and remove barriers.
What's included:
- Regular coaching sessions (fortnightly or monthly)
- Short phone/email support between sessions for urgent issues
- Outcome statements, short reports for plan reviews and suggested wording for planners/plan managers
Pricing: Blocks of hours, monthly retainer, or NDIS capacity building hours (self-managed/plan-managed). Private pay options available.
